Attention Parents/Guardians of incoming 7th and 12th grade students this is a SAVE THE DATE notification:On August 7th 2020 from 2-6pm there will be a Drive-Thru Shot Clinic at the Carroll County Health Department. This clinic will be for 7th and 12th Grade students that are required to have the Tdap and Meningitis vaccines.
There will not be a clinic at the school this year so please call the Carroll County Health Department to schedule an appointment prior to this date, 330-627-4866 Ext: 1530. They will need your insurance information when your appointment is scheduled. When you arrive for your appointment, please have the Consent form completed.
